EXCEPTIONAL

-I was ON-TASK and worked PRODUCTIVELY today. I rarely left my seat, I stayed focused while in groups.
SATISFACTORY

-I was MOSTLY on-task and worked productively today. I left my seat once and stayed mostly on task while in groups.
INCONSISTENT

-I was MOSTLY off-task and
UNPRODUCTIVE today. I left my seat frequently and needed to be reminded to sit down. I needed to be refocused during group work.
LACKS EFFORT

-I was NOT on-task and UNPRODUCTIVE today, AND/OR I made excessive trips out of the room or seat, I rarely participated in group work.
PREPARATION

EXCEPTIONAL

I was PREPARED for class with ALL materials: assignments, pencil, math book, math notebook, and homework. I was on time for class.
SATISFACTORY

I was MOSTLY prepared for class with all but one or two materials: assignments, pencil, math book, math notebook, and homework, AND/OR I was tardy for class.
INCONSISTENT

I was NOT prepared for class today. I was missing MOST materials: assignments, pencil, math book, math notebook and homework, AND/OR I was tardy for class.
LACKS EFFORT

I am never prepared for class with ALL materials: assignments, pencil, math book, math notebook, and homework, AND/OR I was tardy for class.
BEHAVIOR

EXCEPTIONAL

I SHOWED responsibility and respect and did NOT display disruptive behavior during class today.
SATISFACTORY

I MOSTLY showed responsibility and respect, but occasionally I needed to be reminded of appropriate classroom behavior today.
INCONSISTENT

I FREQUENTLY displayed disruptive behavior such as: speaking out of turn, interrupting the teacher, derailing conversations, and distracting my classmates. I frequently needed to be redirected by the teacher today.
LACKS EFFORT

I displayed EXCESSIVE disruptive behavior such as: speaking out of turn, interrupting the teacher, derailing conversations, and distracting classmates. My teacher had to remind me many times during class today.
